Health Certificates and Travel for Companion Animals

Traveling with your pet(s) can be complex and time-consuming.  There are animal health requirements that are specific to each destination. Factors to consider may include:

• Meeting the time frames for obtaining a health certificate 
• Updating vaccinations
• Diagnostic testing
• Administration of medications/treatments

To schedule an appointment for travel certificates, you must first complete the Health Certificates and Travel for Companion Animals Form. Once received, we will review it and call you to schedule your appointment(s).
