At Case Veterinary Hospital we enjoy sharing our patients’ photos and stories within our hospital and on the internet. We would love your permission to share your pet's image & a little bit about them. Sharing our client’s stories is simply a fun way to assist us in educating other pet owners about pet health and showing how cute they are!

I hereby grant Case Veterinary Hospital the right to photograph my pet and/or myself and to use & publish those images. By signing below, I confirm that I understand the following statements:

  • My pet's photo, video, or story may be used on Case Veterinary Hospital’s website, Facebook, Instagram or any media site that suits the hospital’s needs.
  • My pet’s photo, video, or story may be used in a physical publication including but not limited to flyers, articles, and newsletters.
  • I can withdraw my consent at any time.
  • I will receive no compensation for my consent.
  • I understand pictures may be copied and used by Case Veterinary Hospital without further permission.
